Several important things in this page regarding MEDICALS, so please read it thoroughly:

THIS APPLIES TO EVERYONE. Please look at the attachments. You'll find one that says Contestant PRO and Contestant AMATEUR. PLEASE either fill it out digitally, if you're savvy enough to do that, or print it out and fill it out. You can submit this back to me via email or drop off at my office. PLEASE download the correct form for your status.

Please complete the appropriate form: Amateurs complete amateur forms, Pro Fighters complete the Pro form. (Download Buttons for each below.) Medicals are everything and must be completed before stepping into the ring. Please ensure to get these taken care of 1 week before the fight. Get these turned in AS SOON AS YOU CAN. Failure to do so will get you scratched from the match.
A big factor is COMMUNICATION. Please stay in constant communication via email or text. If we are not hearing from you, we will recommend you be replaced. We can't continuously chase 20 fighters for their medical records.


Things that will get your medical records kicked back:
No PHYSICIAN signature (nurse signatures are not accepted. Must be an MD)
Wrong Blood Work - Please note the document and the 3 blood tests required
No Fighter Signature - This happens more often than not. Please sign your records where applicable
MOST COMMON: Incomplete medical records - There's only 5 pages. Please get them all taken care of.

IF YOU RECENTLY FOUGHT WITH US, please let us know so that I can review your medicals and confirm/deny they are within the 6 month TDLR allowed range.

This is from TDLR Directly:

"All medicals will need to be completed by an MD or DO only (except for the eye exam), all forms must be either scanned by Email or Faxed to me. Any contestant 36 years old and older must also get an favorable EEG and EKG.

To help expedite the weigh in process and limit the number of people in an certain areas, We are doing Pre-licensing paper work (the paperwork that fighters fill out normally at a weigh in) to be submitted to us prior to the actual weigh in. Each fighter will be required to fill out the CONTESTANT INFO SHEET.

All fighters who need medicals will need to use the Texas Forms, all medicals must be completed by an MD or DO only, exams will only be accepted if less than 6 months old, forms must be completely filled out by a MD or DO in the US. Please make sure No Nurses fill out the Paperwork.

All medicals must be scanned in or faxed in. NO PICTURES of applications will be accepted."
